Adecco Healthcare & Life Sciences is hiring a Travel Registered Nurse – Home Health/Hospice for our healthcare partner in Plains, PA.

The anticipated hourly wage for this position is $55.92 per hour. Hourly wage may depend upon experience, education, geographic location, and other factors.

Position Overview

We are seeking an experienced Travel Registered Nurse (RN) to provide comprehensive home health and hospice nursing services within the community setting. In this role, you will be responsible for delivering skilled nursing care, completing patient admissions and discharges, and ensuring accurate and timely clinical documentation in compliance with regulatory and organizational standards.

The ideal candidate will be highly independent, organized, and experienced in home health workflows, with the ability to manage a full patient caseload across multiple counties while maintaining high-quality patient outcomes.

Key Responsibilities

• Provide skilled home health and hospice nursing visits to patients in accordance with individualized care plans.

• Complete patient admissions and discharges, ensuring accurate assessment and documentation throughout the care episode.

• Document all patient care activities in Homecare Homebase (HCHB) and ensure synchronization within required timelines.

• Maintain productivity expectations while managing a caseload across designated coverage areas.

• Participate in weekly case conferences and monthly staff meetings.

• Ensure timely completion of required education, training, and orientation modules.

• Provide patient and caregiver education related to disease management, medications, and home safety.

• Coordinate care with physicians, interdisciplinary team members, and community resources.

• Monitor patient progress and communicate changes in condition appropriately.

• Maintain compliance with regulatory requirements, agency policies, and quality standards.

• Participate in on-call rotation, including weeknight and weekend coverage as assigned.

• Support continuity of care and promote patient independence and quality of life in the home setting.

Qualifications

Required Qualifications

• Active Registered Nurse (RN) license.

• Previous Home Health nursing experience required.

• Experience with patient admissions, discharges, and skilled nursing visits.

• Proficiency with Homecare Homebase (HCHB) documentation system.

• Strong clinical assessment and documentation skills.

• Ability to work independently and manage time effectively across a geographically dispersed caseload.

• Strong communication and organizational skills.

Preferred Qualifications

• Experience working in both Home Health and Hospice settings.

• Prior experience working independently in a field-based nursing role.

• Strong understanding of home health regulations and productivity expectations.

Physical Requirements

• Ability to travel between patient homes within assigned service areas.

• Ability to stand, walk, bend, lift, and assist patients as needed.

• Ability to transport required clinical equipment and supplies.

• Ability to work in varied home environments.

• Ability to utilize electronic documentation systems in the field.

Schedule

• Monday – Friday, 8:00 AM – 4:30 PM

• On-call rotation: 1–2 weekends per month and at least one weeknight per week

• 40-hour weekly guarantee (36 hours expected productivity requirement noted)

• Mileage reimbursed between patient visits, from first to last visit

• Navy blue pants and gray top required


Pay Details: $55.92 per hour
