Earn $38.54/hour.
Schedule: 40 hours/week; 1st shift, Monday-Friday
Position Details
The full-time registered nurse (RN) plays a pivotal role in our community-based Dual Diagnosis Treatment Team (DDTT), serving Luzerne, Wyoming, Lackawanna, and Susquehanna Counties. The dual diagnosis team takes a compassionate, recovery-oriented approach dedicated to providing high-quality care and unwavering support for individuals diagnosed with both a mental illness and an intellectual disability. The RN will spend their time in the field, directly interacting with consumers in their homes, traveling, and documenting their work.
Duties and Responsibilities
  • Collaborate with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
  • Perform physical health medical monitoring, assessment, etc.
  • Support implementation of intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
  • Complete accurate documentation in the electronic medical record.
  • Participate in shared on-call rotation.
