Job description

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide rapid hospice field visits throughout Palm Beach County. If you are driven to deliver meaningful, patient-centered care during critical moments, this role offers the opportunity to truly change lives-one visit at a time. You will work the following schedule: 7pm - 7:30am and every other weekend

Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.
What you'll Do
The Field-Based Emergent Care Registered Nurse (RN) provides rapid-response, patient-centered clinical care to individuals in home and community settings. You will:
  • Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
  • You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
  • Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
  • Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
  • Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life

What You'll Need
  • Registered Nurse currently licensed in the State of Florida.
  • Current Basic Life Support Certification.
  • One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
  • Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
