RN Hospice Field Visits

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and dependable Registered Nurse (RN) to provide emergent overnight field visits throughout Broward County for hospice patients & families in need of immediate support. This role is ideal for a nurse who thrives in independent practice, stays calm under pressure, and is driven by a sense of purpose, delivering skilled, responsive, in-home care during overnight hours 7:00pm –7:30am and every other weekend.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What you'll do

  • Support and uphold the organization's mission, vision, and values in every interaction
  • Coordinate patient and family care in collaboration with the attending and hospice physicians
  • Perform comprehensive patient and family assessments to identify physical, emotional, and psychosocial needs
  • Develop and manage the Interdisciplinary Group (IDG) Care Plan
  • Ensure timely implementation and coordination of all interventions and services outlined in the care plan
  • Respond promptly and effectively to emergent needs and concerns during field visits
  • Deliver a consistently high level of personalized, compassionate, and engaging care
  • Advocate for patient comfort, dignity, and quality of life at all times
  • Foster trust and reassurance to ensure the highest level of patient and family satisfaction

What you'll need

  • RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education.
  • Registered Nurse currently licensed in the State of Florida.
  • BSN preferred
  • Hospice certification preferred.
  • Current Basic Life Support Certification.
  • One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
  • Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
