) Registered Nurse (RN) Clinical Manager
Job Summary:
Vertus HealthCare is a Medicare Certified home health agency servicing Palm Beach County, seeking an experienced and compassionate RN Clinical Manager. You will be responsible for overseeing clinical operations, ensuring the highest quality of patient care, and maintaining regulatory compliance. With a strong background in home health, you will serve as a mentor and leader to our clinical staff, driving performance, patient outcomes, and operational excellence.
Responsibilities:
  • Coordinates and oversees all direct care patient services provided by clinical personnel, including:
    • Develops, plans, implements, analyzes, and organizes clinical operations
    • Supervises and supports clinical staff, including RNs, LPNs, therapists, aides, and other home health professionals
    • Reviews and approves patient assessments, care plans, and documentation
    • conducts routine case conferences and participates in interdisciplinary team meetings
    • Monitors clinical performance, caseloads, and patient outcomes
    • Provides field supervision and performance evaluations as needed
    • Collaborate with intake, scheduling, and administrative teams to ensure efficient operations
    • Assists in hiring, onboarding, and training new clinical staff
    • Participates in quality improvement initiatives and ensures documentation accuracy
    • Serves as a liaison between clinical staff, physicians, patients, and families
  • Required Experience/Skills:
    • Graduate of an accredited School of Nursing
    • Registered Nurse (RN) license in good standing, required
    • Clinical leadership experience in a Medicare-certified home health agency
    • Strong understanding of PDGM, QAPI and care coordination
    • Passion for delivering high-quality patient-centered care in a home setting
    • Must know OASIS and two years as a Registered Nurse with at least two years of management experience in home care
