Trustbridge Hospice, a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in the Boca area. If you are passionate about patient-centered care and want to make a profound impact in the lives of patients and families, this is your opportunity. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement (One weekend every 6 weeks)

What you'll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

  • Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
  • Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
  • Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
  • Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
  • Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need
  • Registered Nurse currently licensed in the State of Florida.
  • Current Basic Life Support Certification.
  • Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
