Hospice of Marion County RN Care Center

Hospice of Marion County, a part of Empath Health, is seeking a caring and motivated RN Care Center to work with us at our Hospice House in Ocala while providing exceptional care.

Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. As part of Empath Health, we help neighbors with advanced illness live as comfortably and meaningfully as possible, while supporting their families and caregivers with respect, guidance, and dignity.

Schedule: Nights 7pm - 7am - 36 hour per week; Week 1 Wednesday-Friday and Week 2 Thursday-Saturday

What you'll do:

  • Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
  • The RN will identify patient/family goals, develop a Plan of Care with patient, family, IDT, attending physician and/or partnering facility and continually evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager).
  • The RN will demonstrate the ability to administer, monitor and document medications via all routes using age-specific care practices and provide appropriate patient/family teaching
  • The RN will identify age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.
  • The RN will follow palliative care and end of life principles for all interactions and provide compassionate care adhering to policy and guidelines.
  • The RN directs and oversees the care provided by the LPN and Home Health Aide following agency policy and Care Center protocols. The RN collaborates with Care Center nursing leadership re: changes in patient condition, pt/family concerns, nursing staff needs,
  • The RN supports the overall provision of care in the Care Center by adhering to approved staffing ratios and maintaining Care Center competencies. The RN supports the operations of the Care Center by participating in, and supervising others in, disaster preparedness, fire safety, regulatory inspections, and surveys. The RN performs other duties as requested by supervisor.

What you'll need:

  • Education and/or experience: Associate's degree (A.A.) or equivalent from two-year college or technical school.
  • Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting. An equivalent of education and experience may be considered.
  • Licensure: Valid Florida license as a Registered Nurse, Valid Florida State Driver's License
  • Must be able to remain flexible and work at alternate facility where there is a business need for reassignment for one or more shifts at an alternative facility. When this occurs, the colleague is responsible for their own transportation to the alternate facility and at their expense. If mode of transportation is self-driving to that facility, the colleague must maintain an active, valid driver's license and proof of valid automobile insurance, both of which will be placed in the colleague's HR file.
