Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide Continuous Care visits for patients and families in Central Palm Beach County. This role is ideal for an RN who thrives in one-on-one patient care and finds purpose in delivering comfort, dignity, and peace during life's most vulnerable moments. Your shift would be: 2 days - 7am - 7:30pm and every other weekend.

What you'll Do
The Hospice Continuous Care Registered Nurse (RN) provides continuous bedside care, implements individualized care plans, and collaborates closely with the interdisciplinary hospice team to ensure compassionate, high-quality end-of-life care. As a Continuous Care Nurse, you will:
  • Coordinate patient and family care under the direction of the primary physician.
  • Assess and identify patient and family needs, responding with timely interventions
  • Utilize skilled nursing practices-monitoring, interventions, and education-to enhance comfort and quality of life
  • Provide compassionate crisis intervention and emotional support during critical moments
  • Ensure patient and family satisfaction by promptly addressing concerns with empathy and professionalism

What You'll Need
  • Registered Nurse (RN) license, active and in good standing in the State of Florida
  • Current Basic Life Support (BLS) certification
  • At least one year of experience in one or more of the following preferred: Hospice Nursing, Home Health or Public Health Nursing, Oncology Nursing, Medical/Surgical Nursing, Geriatric Nursing
  • Strong understanding of Hospice Nursing principles, including: Case management, Crisis intervention, Communication and teaching skills, Thanatology and end-of-life care needs
  • Basic computer proficiency; EMR experience a plus
  • Reliable transportation, valid Florida driver's license, and required auto insurance
