This full-time position is entirely remote, offering flexibility while maintaining a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM. As an LCSW in a hospice setting, you will utilize telehealth technologies to connect with patients, conducting therapy sessions and assessments from your home office while following strict ethical guidelines to protect patient confidentiality.

• Provide comprehensive psychotherapy services to patients managing mental health conditions, including anxiety, depression, and post-traumatic stress disorder (PTSD), especially within the context of chronic illness and end-of-life issues.
• Conduct thorough assessments of patients’ mental health status by evaluating their emotional, behavioral, and social functioning, leveraging clinical interviews and standardized assessment tools.
• Collaborate closely with interdisciplinary teams, including physicians, nurses, and spiritual care providers, ensuring a holistic and coordinated care approach that addresses both mental and physical health needs.
• Develop, implement, and adjust personalized treatment plans that are informed by evidence-based practices and tailored to meet the distinct needs and cultural backgrounds of each patient.
• Monitor the progress of individual patients and adapt therapeutic interventions based on both patient feedback and clinical outcomes.
• Educate patients and their families about mental health conditions, coping strategies, treatment options, and available community resources to foster resilience and understanding.
• Maintain accurate and timely documentation of all patient interactions and treatment plans using electronic health record (EHR) systems, ensuring compliance with healthcare regulations and best practices.
• Uphold the highest ethical standards of practice, including maintaining confidentiality and securing informed consent from all patients.

Your primary focus will be on patients in hospice care who are grappling with not only the physical repercussions of terminal illnesses but also the profound emotional strain that can accompany such diagnoses. You will play a crucial role in helping patients navigate feelings of sadness, anxiety, and fear while providing support to their families during challenging transitions.

• A Master’s degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• A minimum of 3 years of hands-on experience in providing psychotherapy and mental health services within clinical settings, preferably with experience in hospice or palliative care.
• Exceptional verbal and written communication skills, facilitating strong relationships with patients and collaboration with interdisciplinary teams.
• Proven clinical skills, with experience in conducting comprehensive assessments and developing individualized treatment plans based on empirical evidence.
• Competence in using electronic health records (EHR) and related healthcare management software for patient documentation and care coordination.
• In-depth knowledge of mental health conditions and therapies, particularly those relevant to geriatric and terminally ill populations.
• Demonstrated ability to adhere to ethical and professional standards of practice consistently.

• Experience working within hospice settings or with patients experiencing chronic respiratory or oncological conditions is highly desirable.
• Familiarity with cultural competency practices to effectively serve a diverse patient demographic.
• Prior training in trauma-informed care practices or end-of-life counseling techniques.

The annual salary for this role ranges from USD 90,000 to 105,000, commensurate with experience and qualifications.
