Home Health Marketing Executive

Prime Home Health is a patient-centered ACHC accredited home health agency proudly serving the San Antonio community. We are committed to delivering skilled, compassionate in-home care that helps patients recover, regain independence, and thrive in the comfort of their own homes. We are looking for a driven and relationship-focused Marketing Executive who shares our dedication to connecting patients and families with the care they need.


The Role

As our Home Health Marketing Executive, you will be the face of Prime Home Health across the San Antonio market. You will build and maintain referral relationships with physicians, hospital discharge planners, skilled nursing facilities, case managers, and other community partners - educating them on our home health services and helping connect patients to the high-quality care we provide.


This is a relationship-first role. You will be trusted to represent our mission with professionalism, warmth, and a deep understanding of the home health landscape.


Key Responsibilities

Build and nurture referral relationships with physicians, hospital discharge planners, skilled nursing facilities, assisted living communities, and community organizations

Educate healthcare providers on home health eligibility, Medicare benefits, and the full scope of Prime Home Health's clinical services

Serve as a trusted advocate for patients and families navigating post-acute and home-based care options

Grow the referral network through consistent, strategic outreach and diligent follow-up

Track outreach activities, referral sources, and relationship-building efforts accurately in our EHR (WellSky)

Collaborate closely with our clinical and intake teams to ensure a seamless, patient-centered admission experience from referral through start of care

Support the clinical and admissions team to ensure patients receive timely, high-quality care from first contact through start of care


Compensation & Schedule

Base Salary: $50,000 - $100,000+ annually, based on experience and existing referral network

Performance Bonus: Monthly performance bonus based on individual contributions

Schedule: Monday - Friday, with occasional flexibility for community events

Territory: San Antonio metro area


What You Bring


Minimum 2 years of home health marketing, community liaison, or post-acute sales experience (required - this is a specialized role)

An established referral network in the San Antonio healthcare community (required)

Solid understanding of home health regulations, Medicare eligibility criteria, and PDGM

Genuine passion for home-based care and patient advocacy

Excellent interpersonal and communication skills - you are known for being warm, professional, and trustworthy

Self-directed and organized - you manage your territory like a business

Comfortable with technology including EHR platforms (WellSky experience is a plus), MS Office, and communication tools

Valid driver's license and reliable transportation
