This role is conducted onsite in patients’ homes, providing a unique work environment that allows for flexible interactions and tailored therapy solutions. Our Home Care setting fosters a supportive atmosphere where you can build lasting relationships with patients while addressing their individual rehabilitation needs. You will collaborate with other healthcare professionals, ensuring a holistic approach to care.

As a Physical Therapist, your day-to-day responsibilities will encompass:

• Evaluating patients recovering from injury, surgery, or chronic conditions within a home care environment.
• Developing and implementing personalized treatment plans, utilizing methods such as targeted exercises, stretching techniques, hands-on therapy, and specialized equipment.
• Documenting patient progress and treatment responses meticulously, ensuring accurate records for ongoing care.
• Regularly reassessing and adjusting treatment strategies to align with individual therapeutic goals, enhancing patient recovery and mobility.
• Providing education and training to patients, caregivers, and family members on effective self-care practices and physical therapy modalities.
• Staying updated on industry standards through continuous professional development and participation in workshops.

Your expertise will benefit a diverse patient population including children, adults, and seniors facing various health challenges. By offering in-home physical therapy, you will facilitate a comfortable and familiar healing environment, which has been shown to improve patient outcomes and satisfaction.

The ideal candidate will possess:

• A Bachelor’s Degree in Physical Therapy (Doctorate highly preferred).
• A valid State License to practice as a Physical Therapist.
• Strong written and verbal communication skills to effectively interact with patients and collaborate with healthcare teams.
• Proficient computer skills, including experience with word processing and documentation software.
• Flexibility and the ability to adapt to changing patient needs and schedules.

While prior experience in a clinical setting is beneficial, it is not required. We welcome recent graduates who are eager to learn and grow within our supportive environment. New graduates will receive mentorship to help them transition into practice effectively.
