This role is primarily onsite, as you will travel to patients' homes throughout the Miami area. • Conduct comprehensive evaluations by reviewing patients' medical histories and performing detailed assessments to determine individual therapy needs.
• Develop, explain, and document tailored treatment plans in collaboration with the patients and their families, ensuring their active participation in goal setting.
• Implement treatment protocols effectively, adapting as necessary based on patient progress and changes in their health status, while staying compliant with current healthcare regulations.
• Collaborate with a multidisciplinary team of healthcare professionals to address evolving patient needs and optimize care strategies.
• Document all patient interactions accurately and timely, including initial evaluations, daily notes, interim progress summaries, and discharge plans to create a comprehensive care record.
• Manage your schedule efficiently to meet patient care expectations and contribute to departmental productivity goals.
• Engage in community outreach activities to promote the services of Swift Health Physical Therapy, enhancing our presence and fostering connections with potential clients.
• Participate in ongoing professional development activities, including serving as a clinical instructor for students and mentoring junior staff.
• Champion patient experience initiatives, driving access to care and working to remove barriers that may impede treatment.

The physical therapist will primarily serve a diverse population of patients requiring home-based rehabilitation services. This may include individuals recovering from surgery, managing chronic conditions, or dealing with the effects of aging. The focus is on improving mobility, reducing pain, and enhancing the overall quality of life for our patients through personalized, hands-on therapy in their home environments.

• Bachelor's Degree in Physical Therapy or related field.
• A valid physical therapy license in the State of Florida is required.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.
• Minimum of 2 years of experience in physical therapy practice, ideally in a home care or outpatient setting.
• Excellent communication skills and the ability to build trusting relationships with patients and their families.

• Experience working in home care or community-based settings.
• Background in geriatrics, orthopedics, or neurology.

The salary range for this Full-Time Physical Therapist position ranges from USD 100,000 to 120,000 annually, commensurate with experience. Additionally, Swift Health offers a competitive benefits package that includes:
