Position: Chronic Care Management Specialist (Care Navigator)

Location: Salisbury, NC (on-site position working directly from outpatient care clinic)

Schedule: Monday-Friday and occasional weekends

What You'll Do:

- Be a health hero to over 500 patients across North Carolina

- Build lasting relationships through regular check-ins and support calls

- Use state-of-the-art remote monitoring tools to track patients' health in real-time

- Collaborate with our clinical pharmacists to optimize patient care plans

- Educate and motivate patients to achieve their health goals

- Be the friendly voice and listening ear that patients look forward to hearing from
