Job Description 

In alignment with the mission, values, and uncompromising standards of Gallaher Signature Living (GSL), the Health & Wellness Director provides clinical leadership and oversight of resident care services in accordance with Title 22 regulations, physician orders, and best practices in senior living healthcare. This role is central to ensuring residents experience a lifestyle defined by dignity, independence, discretion, and exceptional personalized care. This role requires a valid LVN or RN license in the state of California.

Serving as both a clinical expert and trusted partner, the Health & Wellness Director collaborates closely with residents, families, physicians, hospital systems, and community partners to support the physical, emotional, and psychosocial well-being of each resident. The position reflects Gallaher Signature Living’s commitment to a boutique, hospitality-driven environment where wellness services are delivered with professionalism, warmth, and attention to detail.

The Health & Wellness Director monitors, evaluates, and supports resident care plans, ensuring compliance with Title 22, the Nurse Practice Act, and all applicable state and local regulations, while upholding the refined service standards and culture of Gallaher Signature Living.

Key Responsibilities

  • Oversee and coordinate resident care services in compliance with Title 22 regulations, physician orders, and Gallaher Signature Living policies and procedures.
  • Perform nursing duties within the scope of the Nurse Practice Act, including medication administration and treatments as prescribed.
  • Monitor residents’ health status, proactively identify changes in condition, and communicate promptly with physicians and healthcare professionals.
  • Ensure accurate, timely, and comprehensive documentation of resident care, incidents, changes in condition, and required regulatory reports.
  • Maintain medical records in accordance with Title 22 documentation standards and best practices.
  • Follow Universal Precautions and infection control policies to ensure resident and staff safety.
  • Serve as a professional liaison with physicians, hospitals, discharge planners, pharmacies, dental providers, state agencies, and community healthcare partners.
  • Coordinate incidental medical and dental care services with discretion and efficiency.
  • Partner with the Executive Director, Seasons Program Director, and/or Memory Care Director on physician orders, pharmacy coordination, and medication management systems, including central medication storage and delivery.
  • Provide leadership, guidance, and clinical oversight to Care Managers and Medication Aides; step into direct care or medication support roles as needed to maintain service excellence.
  • Support residents with activities of daily living (ADLs) as needed, including grooming, dressing, bathing, toileting, feeding, transferring, and mealtime assistance, while always promoting independence and dignity.
  • Foster an environment that prioritizes privacy, respect, warmth, and emotional well-being.
  • Assist new residents and families with orientation, ensuring a seamless and welcoming transition into the community.
  • Encourage resident engagement in daily routines and lifestyle programs that support wellness and independence.
  • Collaborate with the Lifestyle Department to support curated programs and meaningful experiences.
  • Demonstrate tact, discretion, and professionalism in all interactions with residents, families, visitors, and team members.
  • Respond promptly to resident and staff needs; carry required communication devices while on property.
  • Promote consistent use of communication tools, including shift reports and communication logs, to ensure continuity of care.
  • Assist with staff orientation, training, and ongoing education as requested.
  • Attend required in-service trainings and continuing education to remain current with regulations and best practices.
  • Support community marketing efforts by representing Gallaher Signature Living with poise and professionalism during tours and public interactions.
  • Conduct daily and nightly rounds to maintain a safe, secure, and impeccably maintained environment.
  • Foster a culture of collaboration, accountability, and service excellence across all departments.
  • Uphold the refined image of Gallaher Signature Living through professional appearance, conduct, and communication.
  • Other duties as assigned

Requirements

  • Current Registered Nurse (RN) or Licensed Vocation Nurse (LVN) license in good standing
  • Demonstrated knowledge of Title 22 regulations and healthcare compliance in a residential care setting
  • Prior experience in a healthcare environment; senior living, assisted living, or memory care experience strongly preferred
  • Experience working with older adults, including those with cognitive impairment
  • Strong clinical assessment, documentation, and care coordination skills
  • Exceptional communication and interpersonal skills with physicians, residents, families, and interdisciplinary teams
  • Compassionate, discreet, and service-oriented approach aligned with a boutique hospitality environment
  • Ability to remain calm, thoughtful, and solution-focused in high-pressure situations
  • Ability to meet the physical demands of the role
  • Good physical health verified by required health screenings per state regulations
  • Ability to read, write, and speak English clearly; ability to explain duties and procedures to staff
  • Ability to follow oral and written instructions precisely
  • Current First Aid and CPR certification
