Position: Patient Transfer Center RN
Shift:Full- time 0.9 FTE Night Shift 7p-7a - Every 3rd weekend with rotating Holidays
IU Health is Indiana's largest and most comprehensive healthcare system, and the #1 ranked healthcare provider in the state. We are seeking a dedicated Patient Transfer Center RN to evaluate, coordinate, and process patient transfers into and out of the IU Health System, ensuring seamless communication and optimal patient care during transfer and transport.
Position Overview:
The Patient Transfer Center RN provides expert coordination for patient transfers, collaborating with healthcare providers, facilities, and families to ensure appropriate level of care and timely transport. This role involves obtaining accurate medical and demographic information, communicating effectively with physicians and departments, and managing all components of patient transfer and transport to support safe, efficient, and patient-centered care. The RN will also coordinate bed assignments for scheduled and unscheduled admissions and transfers, contributing to operational efficiency within the system.
Key Responsibilities:
• Evaluate, coordinate, and process patient transfers into and out of the IU Health System.
• Obtain and accurately document medical and demographic information for transfer purposes.
• Communicate effectively with referring and receiving physicians and healthcare facilities.
• Coordinate all aspects of patient transfer, transport, and care needs during the process.
• Engage appropriate departments to facilitate timely transfers and transports.
• Assign beds for all scheduled and unscheduled admissions, including transfers and transports.
• Collaborate with multidisciplinary teams to ensure seamless patient care during transfer.
• Use critical thinking skills to solve problems, resolve conflicts, and prioritize tasks independently.
• Maintain accurate documentation of transfer activities and communications.
• Support a patient-centered approach, promoting safety, efficiency, and professionalism.
• Engage in ongoing professional development and stay current with relevant certifications and best practices.
Qualifications & Requirements:
• Active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
• Minimum of 5 years of experience as a Registered Nurse.
• At least 3 years of critical care, emergency department, or flight nursing experience or equivalent.
• Case management or resource management experience is a plus.
• Exceptional communication, listening, and written skills.
• Strong interpersonal, organization, leadership, and teamwork skills.
• Excellent critical thinking and autonomous problem-solving abilities.
• Proficiency with computer systems and documentation.
• BSN preferred.
• Graduated from an ACEN or CCNE-accredited nursing program.
• Current certifications: Basic Life Support (BLS), Advanced Life Support (ACLS), Pediatric Advanced Life Support (PALS) through the AHA.
• Additional certifications may be required per department policies.
• Pediatric Experience Strongly Preferred
