Job description

NURSE PRACTITIONER (NP) - IN-HOME HEALTH RISK ASSESSMENTS

Position: PRN / 1099 Contractor
Exclusions: PMHNP, Women's Health, Pediatric NP's and PA's.
Schedule: Flexible (50+ hours/month)
Compensation: $100-$125 per visit (Tiered based on availability)

We are seeking Nurse Practitioners (Certain NPs only; no PAs) with no less than 40 hours per month who can start immediately completing in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Must be able to complete credentialing paperwork, training, and onboarding within the first 10-12 days and start field-based work immediately after thereafter.

This is a pure assessment role: No prescribing, no on-call, and zero follow-up care.

THE PAY BREAKDOWN

  • 50 hrs/mo: $100/ visit
  • 70 hrs/mo: $110/ visit
  • 80 hrs/mo: $115 / visit
  • 90 hrs/mo: $120/ visit
  • 100 hrs/mo: $125 / visit
  • Additional Pay: $250 flat-rate paid training, $25 full HRA no-shows, $65 diagnostic-only visits, $10 diagnostic no-shows.Fast-Track Bonus: Extra incentive bonus if you complete onboarding paperwork within 24 hours!
WHAT YOU'LL DO
  • Conduct in-home HRAs (medical history, lifestyle, basic physical/mental evaluations).
  • Provide preventive care education.
  • Document visits using a company-provided iPad.

COUNTY COVERAGE AREAS (Must be willing to cover 2.5 counties, but not all in the same day.): Erie, Henry, Lucas, Ottaway, Wood.
