We are currently seeking Licensed Practical Nurses (LPNs) who are passionate about making a difference and growing their careers in a collaborative healthcare environment.
About the Role
The Licensed Practical Nurse (LPN) plays a vital role in delivering safe, efficient, and patient-centered care across Community Health Net locations. This position includes a $3,000 sign-on bonus, payable in accordance with company policy and contingent upon meeting all employment requirements.
LPNs may be required to float between CHN locations based on operational needs.
Key Responsibilities
  • Conduct and accurately document patient health histories and assessments
  • Assist medical providers in delivering high-quality, patient-centered care
  • Manage patient flow, including triage, follow-up care, and care coordination
  • Coordinate referrals to specialists and community-based services
  • Provide health education to patients and caregivers
  • Collect and process laboratory specimens (e.g., blood, urine, sputum)
  • Support infection control practices, safety standards, and appropriate use of supplies
  • Assist with front office functions as needed (scheduling, patient discharge, etc.)
  • Float between Community Health Net locations as required.
Qualifications
  • Active Pennsylvania Licensed Practical Nurse (LPN) license
  • Current CPR certification
  • High school diploma or GED
Required Skills & Competencies
  • Strong clinical assessment and organizational skills
  • Excellent verbal and written communication abilities
  • Knowledge of medical terminology and basic insurance types
  • Proficiency with computers and electronic health record (EHR) systems
