As a home health care case manager, your job is to provide hands-on patient care within a patient’s home, coordinate the services of other physicians or health care providers, communicate with health insurance providers, and supervise other nurses who visit the home. Your responsibilities also include developing a Plan of Care (POC) for patients based on their needs and feedback from their families, helping support adherence to the treatment plan, monitoring the patient’s needs, including wellness, medical, and specialist treatment, and recommending any community services required. Your additional duties as a home health care case manager include coordinating the implementation of the plan with the primary doctor, making changes as needed, and completing all required documentation.
