Central Coast Salary Range - $97,534 - $141,424

While candidates from anywhere in California are welcome to apply, there is a strong preference for those who reside on the Central Coast (Ventura, Santa Barbara, San Luis Obispo, Monterey and Santa Cruz Counties). This role may offer opportunities for remote work; however, familiarity with and proximity to our local customers is valued.


Job Summary

The Quality Department is dedicated to improving health outcomes by enhancing the quality, safety, and equity of care delivered to our members through strong collaboration with providers and community organizations.

Reporting to the Population Health Manager, the Population Health Supervisor leads the Population Health team in the development, implementation, and monitoring of population health programs and quality improvement activities that advance health equity and improve health outcomes. The Population Health team plays a key role in identifying, prioritizing, and executing plan-wide initiatives aimed at reducing disparities and improving the health of the communities served by CenCal Health.

The Population Health Supervisor is responsible for overseeing daily team operations, including managing productivity, providing staff training and mentorship, addressing performance issues, and identifying opportunities for process improvement. This role supports the development of population health programs and initiatives and facilitates completion of CenCal Health’s Population Health Management (PHM) Strategy and Quality Improvement Health Equity Transformation Program trilogy documents. The Population Health Supervisor supports the ongoing oversight and success of CenCal Health’s Value-Based Pay-for-Performance program, helping to ensure effective implementation and alignment with organizational goals. 

Additionally, this role also contributes to compliance with the state and federal regulatory requirements and accreditation standards. 

Duties and Responsibilities

  • Supervise the Population Health team, managing time-off requests, ensuring timeliness, and addressing daily issues and concerns.

  • Support staff success by coordinating assignments and coverage to help meet goals, including ongoing training and mentoring for both new and experienced team members.

  • Collaborate with management to conduct annual performance reviews and take appropriate disciplinary actions as needed.

  • Work closely with internal and external stakeholders to develop and implement population health programs, quality improvement initiatives, and health equity interventions.

  • Recommend strategies to enhance CenCal Health’s effectiveness in addressing social determinants of health, health disparities, and quality improvement efforts.

  • Suggest improvements to operational systems to increase the impact of CenCal Health’s population health programs.

Oversee the following:

  • Population Health Programs aimed at improving care and preventing negative outcomes due to health disparities, including submission of formal documentation to regulators.
  • The Quality Improvement and Health Equity Transformation Program trilogy documents.

  • Performance Improvement Projects (PIPs) to demonstrate Plan resource allocation and actions to improve care, including submission to relevant regulators.

  • Improvement cycles to track and report on activities to improve care, including submission of documentation to regulators.

  • Year-round clinical improvement activities, ensuring the timely planning, implementation, and coordination of recurring interventions and programs.

  • Quality Improvement and Health Equity Transformation Program workplan documents.

  • The Value-Based Pay-for-Performance program (Quality Care Incentive Program), including oversight of website updates, Primary Care Provider (PCP) collaboratives, measure updates, and data validation.

  • Develop and facilitate training for interdepartmental staff and the provider network on population health programs.

  • Participate in internal and external audits as required.

  • Serve on various committees, teams, and workgroups as needed.

  • Perform other duties as assigned.


Knowledge / Skills / Abilities

Required: 

  • Knowledge of Medi-Cal/Medicaid managed care as it relates to quality improvement and population health programs, including performance measurement, disparity reduction, and value-based care approaches. 

  • Knowledge of population health and quality improvement program design, implementation, and evaluation, with understanding how these activities contribute to D-SNP Model of Care (MOC 4) objectives and annual performance evaluation. 

  • Knowledge of NCQA Health Plan Accreditation Standards with emphasis on quality improvement and population health management requirements.

  • Knowledge of NCQA HEDIS Technical Specifications

  • Ability to lead effectively in a cross-functional team environment.

  • Ability to participate in occasional in-person meetings with advance notice in support of provider engagement and organizational needs.

  • Ability to inspire high team performance and contribute to building a strong team dynamic.

  • Ability to establish and maintain positive professional working relationships, including colleagues, providers, and community partners.

  • Ability to manage competing priorities and meet deadlines while maintaining accuracy and quality in a fast-paced regulatory environment.

  • Analytical and creative problem-solving skills to interpret data, draw valid conclusions, identify root causes for identified opportunities, and implement improvement actions.

  • Self-motivated to continuously seek knowledge for personal and professional growth.

  • Effective oral and written communication skills, including presenting information to diverse audiences.

  • Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint.

Preferred: 

  • Experience with Medicare and/or D-SNP quality and population health programs.

  • Familiarity with State and Local social service agencies and community-based organizations.

  • Bilingual in Spanish.

 

Education and Experience

Required: 

  • Bachelor's Degree in a relevant field.

  • At least five (5) years of experience in population health or quality improvement, or a combination of relevant academic, professional, or work experience.

  • A minimum of one (1) year of experience in a lead, supervisory, or project management role.

Preferred:

  • A graduate degree in a healthcare or management-related field (e.g., MBA, MPH).

  • Experience in Medicare.

  • Experience partnering with community-based organizations to address social determinants of health.

  • Experience with electronic medical records systems, care management platforms, or health plan analytical tools.

  • Experience performing medical records review chart audits
