Job Overview
Visiting Nurse Health System is hiring an MDSHC Coordinator for CCSP who coordinates care for a caseload of patients, ensuring care meets quality standards and utilization guidelines. The MDSHC Coordinator provides upfront review of admission documentation for submission of client assessment data and supplemental medical records into the Georgia Medical Care Foundation (GMCF) online portal. Keys and inputs client data into the GMCF online portal daily. Acts as resource to clinical staff regarding assessment documentation. Provides education and training to CCSP Leadership and clinical teams regarding quality issues, denial reasons and documentation standards based on trends resulting in technical denials from GMCF. Communicates with other waiver programs as necessary to update on client status regarding current waiver services. Tracks denials and appeals internally using Excel spread sheet, communicating updates to appropriate staff. Work in collaboration with the Level of Care Coordinator, works to contact the client's primary care physician contact to obtain signed Levels of Care in a timely manner before they expire. This is an office-based clinical position with minimal opportunities to provide support in the field. There are times when it is required and necessary for this position to make field visits to clients to determine if they meet the level of care required for the Community Care Services Program (CCSP).
What You'll Do:
Quality Review
  • Identifies any corrections to the client's record that are needed/required and works with the individual clinician to ensure corrections are done completely and timely. (Within 24 hours of request for correction)
  • Provides education= and training as required regarding clinical documentation requirements to clinician.
  • Assists clinician supervisors in validating staff performance and compliance with clinical policy and procedure related to the care and services to the client.
  • Reviews admission, comprehensive assessment and visit notes.
Clinical Resource
  • Provides feedback to Supervisor and/or members of the CCSP Leadership team regarding clinicians who are non-compliant with correction procedures.
  • Tracks and makes recommendations for clinician performance improvement to the clinician's Manager based on results of clinical reviews.
Direction
  • Develops staff to independent level of functioning. Progressively counsels' staff who do not comply with VNHS policy and practices.
  • Actively participates in performance improvement process with Management.
  • Demonstrates effective time management skills.

Regulatory
  • Maintains knowledge of GMCF requirements and promotes compliance.
  • Demonstrates knowledge and understanding of Medicare and Medicaid regulations and standards of care.
Quality of Work
  • Makes suggestions for improvement in processes/systems to supervisor; implements changes in processes as directed.
  • Has established a track record of producing work that is highly accurate, demonstrates attention to detail and reflects well on the organization.
  • Is personally committed to high quality of work and encourages others to have similar standards.

Location: Office Location will be at our Hartsfield office in Atlanta, GA
You're Perfect for the Position if you:
  • Graduated from an accredited nursing program (Diploma, Associate, or BSN Required).
  • Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
  • Possess a valid driver's license, reliable transportation, and current auto insurance.
  • 1-2 years of recent nursing experience. Preference given to Home Health, Long Term Care and/or MDS experience.
  • Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
  • Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment.
  • Are proficient with computers and basic technology for documentation, email, HR systems, and online training.
