Community health organization with multiple clinics throughout Delaware.

Job Description:

This is a patient-facing, administrative support role focused on helping patients navigate the healthcare system. The Healthcare Coordinator plays a critical role in managing referrals, authorizations, and ensuring patients can follow through on their care plans.

Key Responsibilities:

  • Handle patient referrals and coordinate with providers
  • Manage prior authorizations for services
  • Communicate with patients by phone to resolve barriers to care
  • Support patients facing challenges such as lack of insurance, transportation, or language barriers
  • Collaborate with internal departments like the enrollment team
  • Track care coordination to ensure follow-through and “close the loop” on services

Qualifications:

  • High School Diploma or GED (required)
  • Experience in healthcare, insurance, or patient service
  • Bilingual in Spanish/English (required)
