Join our team as a Care Manager Extender in Plymouth, NC, where your work directly helps members access the care they need; medical, behavioral health, and community resources. This remote-friendly role combines coordination, outreach, and documentation to make sure no member falls through the cracks. If you're organized, compassionate, and ready to support a dynamic care team in the community, this role is for you.

What you'll do
  • Support care coordination and follow-ups

  • Schedule appointments and manage referrals

  • Conduct outreach to members, families, and local providers

  • Enter and maintain accurate documentation in the EHR

  • Help connect members to medical, behavioral health, and community resources

What you won't do
  • No independent billable case management

  • Work under supervision of a Care Manager or Supervisor
