Job description

Full-time Geriatric Nurse Practitioner (NP) Opening – Southbury, CT  

Affiliated with Danbury Hospital, Nuvance Health Medical Group is seeking an experienced full-time Geriatric Nurse Practitioner (APRN) for SNF and ALF access in the Danbury area at River Glenn Health Care Center in Southbury, CT Monday – Friday plus Call coverage (phone) every other weekend. 
 
River Glen Health Care Center provides compassionate care and the care continuity planning patients need to return to full health after discharge providing expert care for a variety of conditions, long-term assistance or short-term recovery. A three-month mentorship program is part of your initial training experience. 
 
Job Responsibilities:
The APRN provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses.
 
Job Requirements:
  • This position requires a minimum formal education or master’s degree, NP, State of CT RN and APRN license, CT Controlled Substances and Federal DEA Registration and Board-Certified Nurse Practitioner. AGNP preferred but willing to consider FNP candidates with geriatric experience.
  • At least three years’ experience as an APRN in Primary Care/Geriatrics is required.
  • Availability to provide telephone call coverage every other weekend.
