Elcor is recruiting for a full-time Social Worker. Job duties include providing social services for residents, acting as a liaison between medical staff, families, and outside agencies. Interviews residents and families for social histories, Ensures that social and personal aspects of resident care are properly managed, collects and updates information on community health and welfare resources, documentation for care plans, assist with admissions and discharges, conduct care plan & discharge meetings, and additional duties as assigned. Bachelor's degree in social work or closely related field required and experience in long term care preferred, familiarity with Point Click Care preferred.
