About the Role

Are you a compassionate and experienced social work professional looking for a new challenge? We are seeking a Director of Social Work to join our team at a state-of-the-art facility dedicated to providing exceptional care to our residents. As a Director of Social Work, you will play a crucial role in ensuring that our residents receive the highest level of social support and care. If you are passionate about making a difference in the lives of others, we encourage you to apply.

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct the Social Work Department to ensure that the medically related emotional and social needs of our residents are met and maintained.

  • Coordinate and participate in family and resident activities designed to promote social interactions, reality orientation, and intellectual stimulation.

  • Develop one-to-one relationships with residents and families as needed for counseling.

  • Counsel residents and families in dealing with feelings about death or dying and other emotional, mental, environmental, or physical limitations.

  • Complete the social services portion of the Minimum Data Set (MDS) and document the social care component of the Comprehensive Care Plan for each resident.

  • Participate in admission, transfer, and discharge processes, ensuring that all necessary paperwork and documentation are completed in a timely and accurate manner.

  • Collaborate with other departments, including Activities, to provide a comprehensive care plan for each resident.

  • Provide education and training to staff on social work services and best practices.

Requirements

  • Master's degree in social work or human service-related field.

  • At least 2 years of experience in a social work or related field.
