Elan Assisted Living is seeking a dedicated and experienced Memory Care Manager (LPN or RN) to lead and oversee our specialized memory care community. This is an excellent opportunity for a compassionate healthcare professional to make a meaningful difference in the lives of residents with memory impairments while working within a supportive and growth-oriented environment.


Key Responsibilities:

- Develop, implement, and oversee individualized care plans for residents with memory impairments

- Supervise and coordinate daily activities of the memory care team, ensuring high-quality resident care

- Monitor residents’ health status and respond promptly to changes or concerns

- Ensure compliance with all state and federal regulations, as well as facility policies and procedures

- Provide training and support to staff on best practices in memory care and resident safety

- Foster a warm, respectful, and engaging environment for residents and their families

- Collaborate with healthcare providers, families, and team members to optimize resident well-being


Skills and Qualifications:

- Valid LPN or RN license in good standing

- Proven experience in memory care, assisted living, or a related healthcare setting

- Strong understanding of dementia and other memory-related conditions
