Nurse - Memory Care
RN: Registered Nurse or LPN: Licensed Practical Nurse

What You'll Do as an Nurse:

  • Administer Medications and Treatments: Safely deliver prescribed medications and treatments in accordance with physician orders, regulatory standards, and organizational policies.
  • Monitor and Document Resident Conditions: Observe residents, accurately document findings, and communicate changes in condition to the appropriate healthcare team members.
  • Support Care Planning and Admission Processes: Collaborate with RNs and interdisciplinary teams in developing, implementing, and updating individualized care plans, including during admissions and readmissions.
  • Participate in and Oversee Resident Care Programs: Actively support resident care initiatives (e.g., skin integrity, pain management) and ensure CNAs fulfill their responsibilities within these programs.

Nurse Requirements:

  • Active Wisconsin Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
  • Current CPR certification
  • Long-term care experience preferred
  • Basic computer skills, including electronic health records systems
  • Ability to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.
