Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. In this role, youโ€™ll coordinate and oversee patient care in the home setting, ensuring personalized, high-quality outcomes for every client.

What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patientโ€™s physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.
Hours: Full-Time
Paid Drive Time - Company Vehicle
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • No Weekends or Holidays
  • Limited on-call rotation

Qualifications / Job Requirements:
  • Active Wisconsin Registered Nurse License
  • Two (2) yearsโ€™ nursing experience, 1 year in Home Health (preferred)
  • CPR Certification
  • A valid Wisconsin Driver's License and Car Insurance
