Job description

Do you ever get tired of being stuck inside four walls and just clocking in and clocking out every day? Well hospice nursing may be the change you are looking for! Hospice nursing provides a lot of autonomy and independence. You get to be out in the field going in and out of different facilities and homes. You have time to recharge in your car between visits. And most importantly, you get to make a difference to people who need it most...those who are facing a terminal illness and their loved ones.

We are a well-established hospice company looking to add to our growing team!! Preferred Hospice has 5 offices across the state of Missouri covering over 65 counties total.

We are currently looking for a Full Time RN Case Manager to serve patients in the Dexter, MO area. We can do part time or PRN if needed.

The hospice nurse spends most of their work day in their car driving to see patients within a set geographical area. Some required meetings do occur at the office on occasion. This job affords a lot of autonomy as well as great team work.

DUTIES:

  • Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home.

  • The RN Case Manager performs a head-to-toe assessment, provides symptom management and skilled care to patients facing terminal illnesses as well and providing emotional support and education on death and dying.

  • The RN Case Manager supervises LPNs and CNAs, monitors medication lists and appropriateness of said mediations, assesses for DME needs, provides wound care and other treatments as needed.

QUALIFICATIONS:

  • The successful candidate must be currently licensed in the State of Missouri as an RN and live with in a commutable distance to the office.

  • Willing to train the right candidate. A gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness. Must be able to work as a team player and represent our company well in the community.

  • Computer and smart phone skills for EMR charting, email, apps for mileage computation, etc.

  • The offices are open Monday through Friday, 8am to 5pm. Reliable transportation and car insurance required.

  • Health, dental, vision insurance available for Full time along with 401K and optional coverage including cancer policies, short-term disability, etc.

  • Hourly rate plus mileage reimbursement.
