About the Role
As a Registered Nurse (RN) Case Manager, you will oversee resident care delivery within a community-based healthcare setting for the underserved. You will manage medical and behavioral care coordination, support interdisciplinary collaboration, and ensure high-quality, person-centered care with the support of the staff and both security and behavioral health teams. This role also includes operational oversight during evening hours and participation in on-call support as needed.
What you will be doing:
  • Lead care coordination efforts for assigned residents, including medical, behavioral, and daily living support needs.
  • Conduct physical assessments, wound care, medication support, and equipment coordination.
  • Complete admission processes and perform ongoing resident assessments.
  • Develop and manage individualized care plans and service agreements in alignment with regulatory guidelines.
  • Facilitate resident care conferences and serve as a liaison between residents, families, and healthcare providers.
  • Collaborate with interdisciplinary teams including case managers, healthcare providers, and external agencies.
  • Support quality improvement initiatives by tracking and analyzing performance metrics.
  • Provide on-call RN support and guidance to staff during after-hours shifts.
  • Ensure accurate documentation and compliance with electronic health record systems.
  • Maintain adherence to state and federal healthcare regulations.
  • Conduct offsite assessments in various community and healthcare environments.
  • Serve as Manager on Duty during designated evening hours.
About You
You are a compassionate and detail-oriented Registered Nurse who is experienced in coordinating care for individuals with complex medical and behavioral needs and wont shy away from supporting or handling complex situations for residents. You are comfortable working in dynamic and complex environments, collaborating with multidisciplinary teams, and supporting vulnerable populations through long term patient-centered care.
Required Qualifications:
  • Active Registered Nurse (RN) license in Oregon.
  • Experience supporting individuals with substance use disorders and/or sever mental illness.
  • Experience in behavioral health, community-based care, or similar healthcare settings.
  • Strong assessment, care planning, and case management skills.
  • Ability to manage multiple priorities and make informed clinical decisions.
  • Excellent communication and interpersonal skills.
  • Experience working with individuals demonstrating challenging behaviors.
  • Proficiency in electronic health records and case management systems.
  • First Aid, CPR and ACLS certification.
  • Ability to participate in on-call rotations as needed.
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
  • Knowledge of Oregon community-based care regulations.
  • Certifications such as PMH-BC, CARN, or CCTP.
  • Experience collaborating with interdisciplinary healthcare teams.
