As a Licensed Mental Health Counselor, your essential duties will involve:

• Conducting assessments of patients' psychosocial needs and emotional health to create tailored care plans.
• Providing supportive counseling services that align with your clinical training and state-issued license.
• Facilitating goal-directed therapeutic interventions for patients on dialysis and those considering transplantation.
• Collaborating with an interdisciplinary care team to monitor patient progress and adjust treatment plans accordingly.
• Offering patients referrals to community resources such as vocational rehabilitation and home health services, aiding their overall treatment journey.
• Educating clients about advance directives and facilitating conversations surrounding end-of-life care decisions.
• Utilizing established social work theories and practices to identify barriers in patient treatment pathways and develop strategies to overcome them.

Your primary focus will be on adults facing chronic health challenges, including those undergoing dialysis treatment and transplant evaluations. You will engage with patients from diverse backgrounds, each with unique psychosocial needs requiring your attentive listening and empathy. Understanding the interplay between mental and physical health is crucial as you support clients through significant life transitions and emotional turbulence.

• A Master's degree in Psychology, Counseling, Social Work, or similar disciplines.
• Possession of an active Social Work License (LMHC, LMFT, LICSW) as required by state regulations.
• No history of medical malpractice claims, subject to verification.
• Authorization to work in the U.S.
