Job Summary:
We are seeking a skilled and experienced Occupational Therapist (OT) to join our team as a Full-Time Home Health Therapist. The ideal candidate will have experience in home health and be able to provide quality care to patients in their homes.
Responsibilities:
  • Assess patients' needs and develop individualized treatment plans
  • Provide therapy services to patients in their homes
  • Monitor patients' progress and adjust treatment plans as needed
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
  • Provide education to patients and their families on therapy techniques and home exercises
Requirements:
  • Valid Occupational Therapist (OT) license
  • Experience in home health
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to travel to patients' homes
