Salary Range: The anticipated salary range for this full-time position is $65,000 to $85,000 annually. Final compensation will be determined based on education, certifications, directly relevant experience, and demonstrated competency in health informatics, clinical quality reporting, population health, and FQHC operations.

Position Summary

Central North Alabama Health Services, Inc. is seeking a qualified, detail-oriented Health Informatics & Quality of Care Specialist to support clinical quality, data integrity, reporting, and performance improvement initiatives within a Federally Qualified Health Center environment. This position is responsible for strengthening the accuracy, reliability, and use of health information to support high-quality, patient-centered care across medical, dental, pharmacy, chiropractic, behavioral health, X-ray, and laboratory services.

Essential Duties and Responsibilities

• Collect, validate, analyze, and report clinical quality, operational, population health, and service-line data from the electronic health record and related systems.
• Support quality improvement initiatives associated with Uniform Data System reporting, clinical quality measures, patient outcomes, access to care, care gaps, chronic disease management, preventive screenings, and organizational performance goals.
• Monitor data accuracy, documentation completeness, and reporting trends across medical, dental, pharmacy, chiropractic, behavioral health, X-ray, and laboratory service lines.
• Prepare routine and ad hoc reports, dashboards, scorecards, and presentations for executive leadership, clinical teams, quality committees, governing board meetings, audits, grants, and regulatory submissions.
• Develop and maintain quality reports, patient registries, care gap lists, provider performance reports, and population health tracking tools to support proactive patient outreach and care coordination.
• Collaborate with clinical, operational, compliance, information technology, billing, pharmacy, dental, behavioral health, laboratory, imaging, and ancillary staff to improve workflows, strengthen documentation practices, and support reliable data capture.
• Assist with performance improvement projects using recognized quality improvement methodologies, including root cause analysis, Plan-Do-Study-Act cycles, workflow review, process mapping, outcome tracking, and sustainability planning.
• Support organizational readiness for audits, site visits, accreditation activities, HRSA reporting, value-based care initiatives, payer quality programs, grant reporting, and internal quality reviews.
• Provide training and technical assistance to providers and staff regarding quality measure documentation, data entry standards, reporting requirements, workflow updates, and appropriate use of health information systems.
• Identify trends, disparities, service gaps, documentation concerns, and opportunities to improve patient care, patient safety, care coordination, patient experience, and operational efficiency.
• Partner with care teams to support patient outreach, recalls, referrals, follow-up tracking, chronic disease registries, preventive care reminders, and closure of gaps in care.
• Assist with the development, revision, implementation, and monitoring of quality improvement policies, procedures, workflows, training materials, and standard operating procedures.
• Participate in quality, compliance, risk management, infection control, patient safety, and population health meetings, as assigned.
• Support data requests related to grants, board reporting, community needs, strategic planning, payer programs, health equity initiatives, and leadership decision-making.
• Maintain confidentiality and comply with HIPAA, organizational policies, information security standards, and applicable federal, state, and grant requirements.
