Position Summary
The Community Health Worker (CHW) plays a vital role in Community Care Connect by serving as a trusted liaison between the community and healthcare/social service systems. The CHW will work directly with individuals and families to address social determinants of health, promote wellness, and facilitate access to medical, behavioral, and social services. This position emphasizes culturally competent care, health education, and community engagement to improve health outcomes and reduce disparities.

Essential Job Functions
  • Conduct outreach to engage individuals and families in underserved or high-need areas.
  • Conduct in-person visits with clients across designated service areas, requiring frequent travel by vehicle. This includes driving to client homes, community settings, and partner agencies to perform assessments, deliver coordination services, and facilitate referrals.
  • Independently travels to client homes and community locations to provide services and support, ensuring all interactions, materials, and information are handled in strict compliance with HIPAA and confidentiality standards.
  • Build trusting relationships with clients through regular communication, home visits, and follow-up.
  • Assess clients' health and social needs and assist in developing personalized care plans.
  • Provide health education on topics such as chronic disease management, preventive care, nutrition, mental health, and wellness.
  • Connect clients to appropriate services, including healthcare providers, housing assistance, food access programs, transportation, and behavioral health services.
  • Assist clients with health benefit exchange navigation.
  • Advocate on behalf of clients to reduce barriers to care and improve service delivery.
  • Track and document all interactions and outcomes in accordance with program requirements and privacy laws (e.g., HIPAA).
  • Collaborate with clinical and social service partners to ensure coordinated care.
  • Participate in team meetings, trainings, and continuous quality improvement initiatives.
  • Serve as a cultural broker to ensure services are culturally and linguistically appropriate.
  • Participate in community meetings with other agencies and organizations regarding the needs of the community
  • Performs other duties as assigned.

Requirements
REQUIRED EDUCATION OR EXPERIENCE:
  • High school diploma or GED required; Associate's or Bachelor's degree in health, social work, or related field preferred.

REQUIRED CERTIFICATIONS AND LICENSES:
  • Acceptable completion of a criminal history background check
  • Completion of a certified Community Health Worker training program (or willingness to complete upon hire).
  • Completion of health benefit exchange navigation training (or willingness to complete upon hire)
  • First Aid/CPR certificate (not required, but preferrable)
  • Valid driver's license in state of residence, or ability to obtain prior to employment.
  • Must have reliable transportation.
  • Must maintain automobile insurance as required by the State of Washington.

*To support operational needs and compliance requirements, this position is expected to be performed within the State of Washington.

Additional wage progression may be available over time under the organization's established compensation structure. The full established pay scale for this position is $21.65 - $57.73 per hour; however, placement above the starting wage range is not available at hire.
