Registered Nurse (RN) โ€“ Home Health Care
Pay: $45.00 โ€“ $50.00 per hour
Job Types: Part-time, PRN
Schedule: Day shift, Mondayโ€“Friday, holidays and weekends as needed
Start Date: ASAP


Overview
We are seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality, in-home nursing care to patients requiring clinical support, monitoring, and education. The ideal candidate is confident in their clinical judgment, comfortable working independently, and committed to delivering safe, patient-centered care in the home setting.


Responsibilities
โ€ข Perform comprehensive patient assessments and ongoing evaluations
โ€ข Develop, implement, and update individualized care plans
โ€ข Administer medications and treatments as ordered
โ€ข Monitor and document vital signs and clinical changes
โ€ข Educate patients and families on disease management, safety, and home care techniques
โ€ข Collaborate with physicians, caregivers, and interdisciplinary team members
โ€ข Maintain accurate, timely documentation
โ€ข Ensure compliance with HIPAA and all regulatory standards


Qualifications
โ€ข Home health experience: 3 years (Required)
โ€ข RN experience: 5 years (Preferred)
โ€ข High school diploma or equivalent (Required)
โ€ข Associate or Bachelor Degree (Required)
โ€ข Active Registered Nurse (RN) License (Required)
โ€ข CPR Certification (Required)
โ€ข Strong communication and critical-thinking skills
โ€ข Ability to work independently and manage time effectively
โ€ข Compassionate, patient-focused, and reliable
