Reliance Community Care Partners has a unique opportunity for a licensed RN to use their gifts and experience while not getting in the way of a healthy balance of work and life! This position offers the opportunity to work from home locally while residing within western Michigan as well as assessments out in the community and the opportunity to work as an essential part of an interdisciplinary health care team. Territory is the Ludington area counties: Mason, Manistee, Lake, Benzie, and Oceana.

What Will You Do?
In this community-based program, the RN OBRA Evaluator is responsible for performing in-person assessments for individuals who may have been diagnosed with a mental health condition and/or developmental disability and may require placement within a long-term care facility. An assessment is completed to determine the level of care, as well as to make recommendations for both nursing and mental health services.

Education and Experience
    • RN licensure in the State of Michigan
