Home Health Care Patient Coordinator

Job Summary:
The Home Health Care Patient Coordinator plays a vital role in ensuring seamless and effective care for our patients. This position is responsible for coordinating all aspects of patient care, from initial intake and scheduling to ongoing communication with patients, families, and healthcare providers. The ideal candidate will be a proactive problem-solver with excellent communication skills and a deep understanding of the home health care landscape.
Responsibilities:
  • Serve as the primary point of contact for patients, families, and caregivers, addressing inquiries and concerns with empathy and professionalism.
  • Conduct initial patient intake, gathering comprehensive medical and personal information to determine appropriate care plans.
  • Coordinate and schedule patient appointments with nurses, therapists, and other healthcare professionals, optimizing schedules for efficiency and patient needs.
  • Facilitate communication between patients, families, physicians, and other members of the healthcare team to ensure continuity of care.
  • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
  • Process and manage all necessary paperwork, including referrals, authorizations, and insurance documentation.
  • Monitor patient progress and identify any changes in condition or care needs, escalating to clinical staff as appropriate.
  • Assist with discharge planning and transitions of care as needed.
  • Resolve scheduling conflicts and patient concerns in a timely and effective manner.
  • Educate patients and families on services available, insurance coverage, and home care procedures.
  • Adhere to all HIPAA regulations and maintain patient confidentiality.
  • Participate in team meetings and contribute to continuous quality improvement initiatives.

Qualifications:
  • High School Diploma or GED required; Associate's or Bachelor's degree in healthcare administration, nursing, or a related field preferred.
  • Minimum of 2 years of experience in a healthcare coordination, patient services, or administrative role, preferably in home health care.
  • Strong understanding of medical terminology and healthcare operations.
  • Proficiency in electronic health record (EHR) systems.
  • Exceptional communication (written and verbal), interpersonal, and customer service skills.
  • Excellent organizational skills with a strong attention to detail and ability to multi-task.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Proficiency in Microsoft Office Suite (Word, Excel,1 Outlook).
  • Compassionate and patient-centered approach.
  • Knowledge of insurance verification and authorization processes is a plus.
