Job description

Actify Home Care is looking for a Speech Language Pathologist (SLP) to join our growing team!

We have an immediate need for a Speech Language Pathologist (SLP) to assist with growing geriatric patient care needs throughout Broward county.

DUTIES:

  • Accept and schedule patients for home visits typically lasting 45 minutes - 1 hour
  • Assess the skilled Speech Therapy needs of typically geriatric, homebound patients who are often being treated for cognitive and dysphagia, dysarthria, apraxia, and aphasia diagnosis.
  • Develop a care plan to be carried out by yourself in coordination with the approved care plan.
  • Document the condition and plan for each patient via electronic visit notes.
  • Practice infection control best practices to ensure the safety of yourself and patients.
  • Communicate patient needs with our care team to ensure all necessary services are provided.
  • Being part of the team you would be an independent contractor. Sign and submit notes on just 1 easy EMR system, in real-time, with any data enabled device.

  • This role allows you to operate as an independent clinician, and we pay $75 per-session, along with ongoing supportive consulting and resources available from all our team. We are here to help you be successful.
