Job description

Become a part of our caring community
The Care Coach evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
The Care Coach:
  • Ensures a positive and healthy prenatal and postpartum experience by providing comprehensive support, education, and resources to expecting mothers

  • Identifies and resolves barriers that hinder effective care

  • Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning.

  • Understands own work area professional concepts/standards, regulations, strategies and operating standards.

  • Makes decisions regarding own work approach/priorities, and follows direction.

  • Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

  • Quarterly travel to Humana corporate office in Louisville, KY

Required Qualifications

  • Must reside in Kentucky or in a county contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia within 15 miles radius.
  • Associate degree or 3+ years of experience in a similar role.
  • 1+ year of maternal-child health experience.
  • 2+ years of experience in a Care Coach or similar role.
  • Experience with health promotion, coaching, and wellness.
  • Proficiency with Microsoft Word, Excel, and Teams.
  • Ability to use a variety of electronic information systems and software applications.

Preferred Qualifications

  • Experience working with Medicare recipients.
  • Experience with electronic case note documentation and documenting in multiple computer applications and systems.
  • Knowledge of community health organizations, social service agencies, and available community resources.

Additional Information

  • Schedule/Time Zone: Monday through Friday, 8:00 AM - 5:00 PM Easter Time Zone with flexibility to work overtime as per business needs.

  • Work Location: Kentucky

  • Work Style: Remote

  • Travel Requirements: 5% travel includes attending face-to-face and collaborative meetings at Humana's Louisville, KY location.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
