ROLE - SPECIALTY
Acute Care Case Management / Discharge Planning
Requirements
- Minimum of 2 years of relevant experience
- California state license required
- Charting experience with Epic
- Experience with HMOs, IPAs, Medicare, Medicaid preferred
Roles & Responsibilities
- Conduct initial assessments within 24 hours of patient admission.
- Coordinate discharge plans and all post-acute care needs.
- Collaborate with medical teams, insurance case managers, and utilization management teams.
- Facilitate safe and timely discharges to appropriate levels of care.
Compliance / Notes
- Orientation: 40 hours
- Required pre-start modules: Approximately 16 hours (non-billable)
- Must be willing to float between facilities within the state.
