Crestwood Behavioral Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team as a Remote Hospice Care Specialist. Key Responsibilities

As a Licensed Clinical Social Worker at Crestwood, you will have the opportunity to focus on delivering high-quality mental health services to patients through the following responsibilities:

  • Conduct comprehensive mental health evaluations and render accurate diagnoses to ensure appropriate treatment.
  • Develop individualized treatment plans that address the unique needs and challenges faced by patients in hospice care.
  • Provide regular psychotherapy sessions, maintaining a manageable caseload while ensuring consistent and effective patient engagement.
  • Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and therapists, to coordinate holistic patient care.
  • Engage in crisis intervention as necessary, providing timely support to patients in distress.
  • Adhere to regulatory guidelines, including HIPAA and CMS standards, to uphold patient confidentiality and care quality.
Patient Population / Client Focus

Qualifications
  • Master’s Degree in Social Work from an accredited institution is required.
