Job Summary:Â
To provide direct services to individuals and families with complex trauma, abuse or neglects, and intimate partner violence issues with the goal of increasing stabilization, safety and well-being. Direct services include assessments, intervention education, and linkage with needed community-based resources and services.Â
Duties and Responsibilities:Â
Assess and refer children/individuals/families to supportive and concrete services, housing, financial assistance, education, substance use, mental health, medical, immigration, legal, etc. Â
Provide case planning, assessment, crisis intervention, establish goals and objectives and counseling services for families who have a child at risk of Foster Care placement. Â
Conduct assessments of families within 30 days of intake and reviewed review and update in 6 months after intake. Â
Participate and attend all ACS, OCFS, Agency and Model Developer training as requested or required. Â
Provide crisis intervention and counseling services to children, teens, individuals and families.Â
Collaborate with program case aid/advocate in aiding and supporting families in obtaining resources, support services, and advocacy assistance.Â
Collaborate with ACS to monitor compliance with mandated services and family attendance of court hearings. Â
Collaborate with ACS legal team to ensure all service needs are being addressed by families.
Document and maintains all mandated tracking logs/excel sheets, case record electronic systems, and document in a timely manner.Â
Complete all program required paperwork, i.e., Progress Notes, Program Family Assessments, Family Assessment Service Plans (FASPs), and other reporting requirements.Â
Request, participate, facilitate and or attend conferences, meetings internal or external to the agency.Â
Assist individuals and families in developing services plans addressing safety, stabilization, substance use, mental health, domestic violence, from a harm reduction approach.
Participate in weekly Supervision, with a planned agenda on family case issues to discuss Implement Quality Assurance data to drive case practice and improve performance and achieve outcomes.Â
Monitor and obtain information on the child/ren school placement and educational performance.Â
Monitor and obtain information on the family's medical needs, including compliance with periodic physicals and immunizations.Â
Requirements
Skills, Knowledge, and Abilities:Â
Ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful, caring manner. Â
Ability to communicate effectively with supervisors, co-workers, and other community partners
Demonstrates the ability to organize and coordinate work within schedule constraints and handle emergent requirements in a timely manner. Â
Ability to conduct interviews with families to obtain and provide the information needed to carry out case management responsibilitiesÂ
Knowledge and ability to use critical thinking and analytical skills used to formulate with the family person achievable plans for implementationÂ
Ability to perform the essential functions of the job with or without reasonable accommodation. Â
Collaboration and facilitation skills to work with the individual, families and services providers. Â
Demonstrates cultural sensitivity and competence in work with families from various ethnic and cultural groups, social-economic status and lifestyles.Â
Qualifications/Requirements:Â
Bachelor's required.Â
Bi-Lingual is preferred but not needed. Spanish, Chinese/Mandarin, Russian, Italian, French Creole. Â
Experience with Microsoft Word, Excel, PowerPoint, CNNX, PROMIS Â
At least two years of child welfare and counseling experience preferred.Â
Due to the nature of the work environment work hours will be based on the needs of program and may varyÂ
Perform other duties/responsibilities as assigned within the scope of the position.
