Physical Therapist (PT) & Physical Therapist Assistant (PTA) – Per Diem / PRN Home Health

Pay:

  • PT: $80 per visit

  • PTA: $60 per visit

Location: O‘ahu, HI (reliable transportation required)
Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

Always Best Care is a growing Medicare-certified Home Health agency serving patients throughout O‘ahu. We focus on fall prevention, mobility, strength, and helping seniors remain safe, independent, and thriving in their own homes.

We are currently hiring licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) who are passionate about providing high-quality, patient-centered home health care.

Responsibilities
  • Provide skilled in-home physical therapy services

  • Complete evaluations, reassessments, and fall risk assessments (PT)

  • Implement individualized treatment plans to improve mobility, balance, strength, and safety

  • Educate patients and caregivers on home exercise programs and fall prevention

  • Document accurately and communicate patient progress with the interdisciplinary care team

  • Maintain compliance with Medicare home health documentation requirements

Requirements
  • Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)

  • Minimum one year of clinical experience (home health experience preferred)

  • Strong documentation, communication, and time-management skills

  • Ability to work independently in the field

  • Valid driver's license and reliable transportation

  • Passion for serving seniors, kupuna, and local families
