Safe At Home Service Facilitation, LLC.
Service Facilitator (Independent Contractor – 1099) - Must live in the Northern Virginia. Face to Face visits required.

The Service Facilitator is responsible for coordinating and monitoring Medicaid Home and Community-Based Waiver services to help individuals safely remain in their homes and communities. This position conducts assessments, develops and updates person-centered Plans of Care, and completes required routine and reassessment visits in compliance with Virginia DMAS regulations. The Service Facilitator monitors each individual's health, safety, and welfare while identifying changes that may require additional services or interventions. The role requires maintaining accurate documentation, submitting required reports on time, and collaborating with members, families, healthcare providers, and community resources. Strong organizational, communication, and time-management skills are essential, as this position requires working independently and managing a flexible schedule. Reliable transportation and the ability to travel throughout Northern Virginia are required.

Job Requirements & Education:
  • High school diploma or GED required.
  • Experience working with individuals with physical disabilities, intellectual/developmental disabilities, or mental health disabilities required.
  • Successful completion of Virginia DMAS Service Facilitation Module Training (or willingness to complete prior to providing services).
  • Knowledge of Virginia Medicaid Waiver programs and person-centered planning is preferred.
  • Excellent documentation, computer, communication, and organizational skills.
  • Valid driver's license, reliable transportation, and the ability to pass all required background screening.
