Every other weekend - Saturday & Sunday, 8-10 visits per day

Responsibilities
  • Provide clinical oversight and support for home health services to ensure high-quality patient care.
  • Conduct supervisory patient visits and quality assurance visits.
  • Assess patient conditions and identify age-specific and diagnosis-related care needs.
  • Review, monitor, and maintain accurate, timely, and compliant clinical documentation.
  • Investigate and resolve patient, family, and employee concerns professionally and promptly.
  • Collaborate with physicians, patients, caregivers, and interdisciplinary healthcare teams to coordinate care.
  • Provide clinical guidance, education, and support to nursing staff and caregivers.
  • Participate in quality improvement initiatives and ensure compliance with company policies and regulatory standards.
  • Utilize critical thinking and sound clinical judgment to resolve issues and improve patient outcomes.
Qualifications
Required:
  • Active and unrestricted Registered Nurse (RN) license in the State of Indiana
  • Minimum of two (2) years of recent home health nursing experience
  • Valid driver's license and reliable transportation
  • Strong assessment, documentation, and clinical decision-making skills
  • Experience with electronic medical records (EMR/EHR)
  • Excellent communication, organization, and leadership abilities

Preferred:
  • Experience with Medicare home health regulations
  • OASIS documentation experience
  • Case management or clinical supervisory experience
  • Bilingual candidates are encouraged to apply
