Bright Path Social Work is seeking a dedicated Senior Licensed Social Worker to join our team on a full-time basis, focusing on hospice support for seniors in Philadelphia, Pennsylvania. The ideal candidate will possess a Master's degree in Social Work and hold an LMSW license in the state. This role is designed to be performed remotely, providing the flexibility necessary to manage schedules effectively while ensuring continuity of care for our clients. The Senior Licensed Social Worker will have a full-time schedule from Monday to Friday, from 8:00 AM to 4:30 PM, with some occasional evening or weekend hours based on client requirements. On-call availability may be expected for urgent situations. Our work setting emphasizes collaboration, and as such, you will work closely with healthcare teams, accessing various digital tools and maintaining a record of services through electronic health record (EHR) systems.

The selected Senior Licensed Social Worker will carry out crucial responsibilities that include:

• Performing comprehensive assessments of the psychosocial needs of seniors, evaluating emotional, mental, and social factors influencing their care.
• Collaborating with clients and their families to design individualized care plans that align with their specific goals, preferences, and life circumstances.
• Providing counseling, emotional support, and resources that assist seniors in navigating the complexities of aging and end-of-life decisions.
• Advocating for client rights and access to essential community resources, ensuring they receive adequate support and services.
• Working jointly with interdisciplinary teams, including nurses and caregivers, to coordinate high-quality services tailored to client needs.
• Regularly visiting clients to monitor their well-being and safety in their living environments and addressing any issues effectively.
• Facilitating communication among family members to resolve conflicts that may arise regarding the care of seniors.
• Educating clients and their families about available programs, support groups, and community resources that enhance their social engagement and quality of life.
• Maintaining meticulous documentation of client assessments, care plans, and progress reports to ensure compliance with legal and ethical standards.
• Staying updated on current social work practices, policies, and regulations to provide high-standard care.

In this role, you will primarily focus on supporting elderly clients receiving hospice care—individuals facing serious, chronic, or terminal health conditions. Your engagement will be pivotal in ensuring that clients receive not only medical care but also emotional and social support critical during challenging times. You will help alleviate family tensions and grief, facilitating a supportive environment for clients and their loved ones.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of experience in a social work role, with preference given to experience in geriatric or hospice care settings.
• Thorough knowledge of social work principles, aging-related challenges, and familiarity with community resources available to seniors.
• Strong assessment, counseling, and problem-solving skills.
• Demonstrated ability to empathize, build rapport, and establish trust with seniors and their families.
• Excellent communication skills, fostering effective collaboration with multidisciplinary teams.
• Familiarity with electronic health record (EHR) systems and proficient use of relevant computer applications.

While two years of experience is required, candidates with experience in hospice care, palliative care, or geriatric social services will be favored. Familiarity with community health resources and additional skills in conflict resolution will enhance your application.
