As a key member of our social services department, you will play a vital role in providing exceptional support to our residents and their families. Responsibilities:

• Provide emotional, social, and psychological support to residents, addressing their physical, emotional, and spiritual needs
• Develop and implement individualized care plans in collaboration with residents, families, and healthcare professionals
• Coordinate and facilitate resident outings, social events, and activities, ensuring a sense of community and social engagement
• Maintain accurate and detailed records of resident care, progress, and interactions
• Communicate effectively with residents, families, and staff members to ensure a seamless care experience
• Collaborate with interdisciplinary teams to address resident needs and concerns

Requirements:

• High school diploma or quivalent required;

  • degree in social work, psychology, or related field preferred

    • Ability to maintain confidentiality and discretion when handling sensitive information
    • Must be able to lift up to 25 pounds and stand for extended periods
