Key Responsibilities

As an LCSW at Life Essence Center, your responsibilities will include:

  • Conducting thorough clinical assessments to understand the unique emotional and social needs of your patients.
  • Developing personalized treatment plans grounded in best practices and solution-focused approaches, enhancing each patient's quality of life.
  • Implementing therapeutic interventions, including cognitive-behavioral therapy (CBT) and mindfulness strategies, tailored to meet the diverse needs of patients facing end-of-life issues.
  • Maintaining accurate and confidential clinical records that adhere to HIPAA regulations, ensuring compliance and ethical standards are met.
  • Collaborating closely with an interdisciplinary team of healthcare providers to ensure seamless and integrated care, participating in regular case reviews to enhance treatment effectiveness.
  • Providing education and resources to patients and their families, empowering them to navigate mental health challenges and understand available support systems.
  • Engaging in continuous professional development through ongoing training and staying informed about the latest advancements in hospice care and mental health.
Patient Population / Client Focus

Your expertise will predominantly support patients in hospice care who are facing terminal illnesses, along with their families. You will address their emotional distress, grief, and existential concerns, helping them find peace and understanding during this pivotal time in their lives. Your compassionate approach will be vital in helping families cope with loss and prepare for the emotional journey ahead.

Qualifications
  • A Master's degree in Social Work from an accredited institution, demonstrating your professional training and background in mental health.
  • A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of New York, ensuring adherence to regulatory standards.
  • A minimum of 3 years of experience in clinical social work, particularly in settings that address grief, loss, and mental health challenges in patients.
  • Proven knowledge and application of evidence-based therapeutic techniques, with a keen understanding of mental health issues relevant to hospice care.
  • Exceptional communication and interpersonal skills, with the ability to establish trust and rapport with diverse populations.
  • Strong problem-solving abilities and critical thinking skills, enabling effective interventions and support to patients in emotional distress.

Preferred Experience
