Job Summary:

The Episode Coordinator is an experienced nurse responsible for effectively coordinating clinical care for patients. The Coordinator proactively identifies and addresses patient needs, ensuring that all individuals receive proper clinical guidance throughout their care within the RWJBH system.They work collaboratively with the care team and multidisciplinary teams to establish and manage care plans across the RWJBH network.

Qualifications:

Required:

  • Associates in Nursing required
  • At least 3 years of relevant clinical nursing experience
  • Experience in program development and evaluation
  • Commitment to maintaining confidentiality of medical records and collaborating effectively
  • Compassionate support for patients and families
  • Initiative and the ability to work independently
  • Strong organizational and time management skills
  • Proficiency in documenting patient care in electronic medical records
  • Excellent verbal and written communication skills
  • Strong problem-solving skills and techniques for team development

Licenses and Certifications Required:

  • Active nursing license RN in New Jersey
  • Basic Life Support BLS certification

Scheduling Requirements:​​​​​​​

  • Physical Working Location(s): This role will support RWJ Somerset
  • Working Hours: M-F 8A-5P

Essential Job Functions:

  • Key responsibilities include hospital rounds, providing bedside education, program planning and evaluation and coordinating appointments, including scheduling
  • Ability to make informed clinical assessments and educate staff and families
  • A strong understanding of disease care standards and essential competencies is crucial,
  • Support care team transitions across various care settings, including moving from active treatment to survivorship or end of life care
  • Serve as a consistent point of contact for patients and their families
  • Act as a liaison between patients, families, RWJBH providers, and other care locations
  • Coordinate patients' care plans with their caregivers and providers
  • Educate patients about their diagnoses, treatment options, side effect management, and the importance of adhering to treatment plans
  • Guide patients on coping strategies, advance directives, and end of life issues
  • Provide psychosocial support and facilitate necessary referrals
  • Manage the scheduling of appointments, diagnostics, and procedures
  • Assess the educational needs of patients and promote communication within the care team
