Job Title: OSH Registered Nurse Address: Chicago Heights, IL Job Description: Support patient assessment and prioritization for telephonic triage to ensure appropriate care and prevent unnecessary hospital visits. Execute protocol-driven chronic condition management. Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care over the phone. Conduct medication reviews within nurse-led visits to ensure patient safety and adherence. Review and act upon abnormal laboratory results to maintain patient safety and continuity of care. Utilize standardized protocols for medication management, prescription renewals, and prior authorizations. Manage clinical communications, ensure task completion, and close patient visit encounters promptly.
