Our mission is to promote independence, dignity, and quality of life for the members we serve while maintaining the highest standards of compliance and care.
Position Overview
Epic Nursing Services is seeking a Case Manager to support members enrolled in the Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs.The Case Manager works directly with members, caregivers, and healthcare professionals to coordinate services, monitor care plans, and ensure that members receive person-centered support that meets MassHealth LTSS program requirements and CARF accreditation standards.This role involves regular member visits, care coordination, documentation, and collaboration with the clinical team.
Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, Public Health, Nursing, or a related field
  • Equivalent experience in community-based healthcare or human services programs may be considered
  • Experience with MassHealth LTSS programs preferred
  • Experience working in Adult Foster Care (AFC), Group Adult Foster Care (GAFC), PCA programs, or home care programs preferred
  • Strong documentation and organizational skills
  • Excellent communication and interpersonal abilities
  • Ability to manage multiple member cases simultaneously
  • Ability to work independently in community settings
  • Cultural competency working with diverse populations
  • Valid driver's license and reliable transportation
  • Ability to travel to member homes within assigned service areas
  • Ability to maintain confidential records in accordance with HIPAA requirements

Key Responsibilities
  • Conduct required member visits in accordance with AFC and GAFC program regulations
  • Monitor member health, safety, and overall well-being in the community
  • Identify and report changes in member condition to the clinical team
  • Coordinate services between members, caregivers, healthcare providers, and community partners
  • Assist members in accessing community resources and support services
  • Support effective caregiver-member relationships and communication
  • Collaborate with the Registered Nurse to support person-centered care planning
  • Participate in service plan reviews and updates when member needs change
  • Maintain accurate and timely documentation in accordance with MassHealth LTSS AFC/GAFC regulations, Epic Nursing Services policies, and CARF standards
  • Ensure all case notes, member interactions, and service updates are documented appropriately
  • Participate in internal chart reviews, audits, and quality improvement activities
  • Support Epic Nursing Services' performance improvement and compliance initiatives
  • Advocate for the rights, dignity, and independence of members served
